Division 1
Everybody who plays in Division 1 of the SBPSL must have a CSAS (California Soccer Association South) players card. To obtain a card, follow the instructions below:

1. Find a team to play on and obtain a CSAS sign up form from the team manager
2. Complete the sign up form
3. Find 2 identical passport sized photos of yourself. The photos should be approx. 1 ½" H x 1 ½" W.
4. Write a $14 check out to CSAS
5. Create a self addressed stamped envelope (SASE).
6. Send SASE, photos, sign up form and check to:

CSAS will create your card and mail back to you in the SASE you provide.

Divisions 2 - 4
Everybody who plays in Division 2, 3 or 4 of the SBPSL must have an SBPSL players card. To obtain a card, follow the instructions below:

1. Download and print the PDF Player Release Form
2. Make a good photocopy of your drivers license. Cut to size and attach to sign up form.
3. Complete the SBPSL Player Release form
4. Find a passport sized photo of yourself. The photo should be approx. 1 ½" x 1 ½" (cut to size).
5. Write a $5 check out to SBPSL
6. Create a self addressed stamped envelope (SASE)
7. Send SASE, photo, photocopy of drivers license, release form and check to:

Mike will create your card and mail back to you in the SASE you provide. You need to get the info to him by Tuesday afternoon in order to get the card back in time for that weekends game.
